REABLEMENT PRACTITIONER

The role of a Reablement Practitioner is to facilitate the rehabilitation of clients who have returned home after being discharged from hospital or another healthcare facility.
Over a period of up to 6 weeks, you will be dedicated to teaching clients essential skills that will assist them in effectively managing their day to day living tasks. Our ultimate objective is to enable our clients to regain their independence, granting them the freedom to live life on their own terms.
As one of our team of dedicated Reablement Practitioners, you’ll be working alongside other professionals in the Health & Social Care sector, including Occupational Therapists, Physiotherapists, Nursing teams, TEC practitioners, and Social Workers.
This role does require assisting with personal care and also requires you to have a full driving license with your own transport. The rotas are scheduled on a 4 on 4 off basis with your working hours being 07:00-14:00 and 17:00-22:00 per working day.
